The cytoskeleton of the cells is a dynamic structure and an organized network of three primary protein filaments: microtubules, actin filaments, and intermediate fibers. It acts as a framework that gives the cell it's shape and:

Acne starts when the shear forces of sebum produced by the sebaceous glands inside hair follicles, during puberty, damage the cells lining the ducts through which it flows to the surface of the skin and our body responds to such lesions with a strong inflammatory reaction.
This just aggravates when the follicles get clogged and bacteria start proliferating within, in the rich feeding medium that is sebum for them, specially for Propionibacterium acnes, the bacteria which are regularly present in the hair follicle.
Fact is before puberty there is no sebum production in the hair follicles.
A strong increase in sebum excretion occurs a few hours after birth; this peaks during the first week and slowly subsides thereafter. A new rise takes place at about age 9 years with the onset of adrenal androgen secretion, and the signs of puberty in boys or girls, including pubic hair, axillary hair, and adult body odor, and continues up to age 17 years, when the adult level is reached.
The increased levels of the male hormone, testosterone (which is present in both males and females), produces growth in the sebaceous (oil) glands of the skin, increasing the production of sebum.
Sebum secreted by the sebaceous glands consists of squalene, free fatty acids, wax esters, and triglycerides. The severity of acne is correlated with volume of sebum produced. The continued production of sebum and keratin gives rise to visible noninflammatory lesions, including the closed comedo (whitehead) and open comedo (blackhead). The contents of a comedo include sebum, cells, hair, and Propionibacterium acnes.
Breakdown of the follicular wall of a comedo causes the rupture of its contents into the surrounding dermis, leading to inflammation. Varying in degree of breakdown, inflammation and confluence, the resulting lesions include papules, pustules, and nodules.
The papule, an inflammatory lesion, results from the complete breakdown of a comedo. On the surface, it is a red, raised, solid lesion, less than 5 mm in size.
Arising from a partial breakdown of a comedo, a pustule is a raised, purulent lesion with a “yellowish” center.
Nodules are deeper and larger than papules, often involving the fusion of 2 or more disintegrated comedones.
The longest diameter of a nodule exceeds 5 mm.
Severe acne involves multiple nodular lesions.
